We are actively recruiting for our Cycle number 14 that starts Janurary 12 2008. 
 
 
Attendance at one of our orientations is MANDATORY. Refer to our listing of orientations for the one nearest to you. The candidate and one parent/guardian must attend together and an interview will be conducted by a staff member of NMYCA. An overview of our academy will be given and any questions will be answered at this time.

At lease one Mentor packet must be completed and submitted to us. Be sure to download the candidate checklist and ensure you include ALL the required copies and complete the notarized forms. Only complete applications will be accepted.

The following file contains all the forms (complete application) in one pdf file. The file is in three parts. CLICK HERE TO DOWNLOAD (PDF). The first part is a National Study series of questionnaires, the second part is the actual application and the final part is a checklist to ensure a compete packet. Print the forms, complete the forms listed on the checklist  and mail the application(s) to:
