Applications are accepted on a first come first served basis and eligilbility. Please submit your pre-application as early as possible.

In order to apply, please fill out this form to receive an application, there is No Application attached, only the Pre-App.  An application will be mailed to you after we receive the pre-app.  Please make sure that you send a copy of your birth certificate when you submit your completed application.  You must be a legal resident of Arkansas and a US citizen.
