Until problems with the new online reporting system are worked out we will continue the previous reporting method.
Instructions for completing your report:
- Type in submission code mcamentorreport
- Complete all areas pertinent to your cadet including all contact dates and placement.
- If no contacts were made a report is still needed with an explanation in comments/notes area.
- If cadet has moved from the area, has enlisted in military or gone off to school, updated information can be gathered from family members.
- If placement includes employment please verify with employer.
- Click on Case Managers and hit submit.
- Click here to go to the online report.
