How do we track our success?
If you have been browsing our website, you can see by now that TYA is geared to help our youth get back on track. Our success rate is impressive, but the honor doesn't belong to us alone. Our Mentors play a very important part in this process. Mentors become an extension of TYA when the cadets have left the Residential Phase of the program.
It is very important to us to know what our graduates are doing after TYA. It is our sincere hope that they will continue to improve upon themselves and pursue their goals. This is where the Mentors become our eyes and ears.
Mentor, Graduate & Case Manager
Each Mentor and graduate will be assigned to a Case Manager for the 12 month Post-Residential Phase after graduation. Mentors and graduates are required to be in contact a minimum of four times per month. Each month the assigned Case Manager will contact mentors and request a Monthly Mentor Report.
Monthly Mentor Report (MMR)
The MMR is our information sheet. It is with this report we see what the graduate has done with the tools she/he has learned. Are they employed? Go back to school? Join the military? How many times were Mentor and graduate in contact? What type of contact? We want to know! This is our feedback to determine whether we are successful at what we do or should we implement some changes.
Reporting Options
As mentioned earlier each assigned Case Manager will contact you with their contact information. The easiest way to correspond with your Case Manager is by E-mail. If that is not an option you can mail, phone or fax a report to:

Newest method - an on-line reporting site which is linked to the cadet life goals system and allows you to submit the report on-line! This site automatically loads the report information into our data management program. With a login ID and password, you can review your cadets goals and submit reports from anywhere you can access a computer!
