Mentor Reports

Mentor Monthly Reports are due by the 5th of the month.  During one of your face-to-face meetings each month, please review the Post-Residential Action Plan.  Is your cadet on track?  Have plans changed?  Please indicate in the Remarks section.  Information give here reflects current involvement, not what they intend to do several months from now.

If you think your cadet could benefit from a contact from one of our staff this month (for a word of encouragement or a to light a fire) please indicate this in the Remarks Section.  Please note any change/additions in your address in the Remarks Section as well.  If the cadet was charged with a law violation, please describe this in the Remarks Section.
