Following are the applications that will be reviewed for acceptance into the academy.  Please note that completion of these applications is for review purposes only and does not guarantee acceptance. 

Acceptance will additionally be based upon an interview, test scores and meeting the basic eligibility requirements. After they have sent in the initial application and have been tested and interviewed, all applicants will be notified via U.S. mail regarding whether or not they have been accepted in the academy.

Here you may download the Initial Application, which must be sent in first. This application will help us determine if you meet the basic eligibility criteria. You may print it, fill it out and return it to the following address:

After you have been tested and interviewed, you may download the Main Application and send it to the same address above. All requested information is required. Incomplete applications will not be considered for acceptance until all paperwork is received.
