There are now two ways to submit your Mentor Report.  You may either go to the Cadet Achievement website to submit Mentor Reports or follow the Mentor Report link and filling out the form. 

Please note that the preferred method is the Cadet Achievement site located at https://achievement.ngcp.org/.
